What do My UltraSounds Mean?!

6/28/07 US: The right ovary appears normal, measuring 3.6 x 2.0cm. Within the left hemipelvis there is a large complicated cystic mass within overall size of 6.1 x 3.6 x 4.1cm. The loculated dominant cystic components show diffuse low level internal echoes. Also within this complex cystic structure or central follicle sized cysts as well as moderately hyperechioc intralesional nodule which is 7x7mm in size. Differential includes endometriosis, hemorrhagic cyst. Alternatives such a s dermoid a cystic malignancy cannot be excluded.

7/19/07 US: The right ovary measures 3.54 x 3.30 x 6.21 cm The left ovary measures 6.55 x 3.62 x 3.90cm. The ovaries are enlarged. The appearance of the left ovary is similar to the prior exam with what appear to single bilobed mass, or two seperate masses, indeterminate in echogenicity between solid and cystic with considerable debris. Ovarian tissue appears stretched and thin around these findings. Follicular type cysts are seen within the ovarian tissue. The right Ovary demonstrates a cystic structure measuring 4.58 x 3.09 x 3.65. It has internal echoes some of which are linear and maybe septae. This was not present previously. There is free fluid within the cul-de-sac.

If you could read my previous post a few days ago along with this and translate the above in basic english I would appreciate it. I'm going in for laproscopic surgery and I'm terrified I have cancer even though he stills says I'm too young--26.

Hi Gothique

thank you for the information. I would guess that your right ovary shows a new cyst of ovulation.

the cyst on your left ovary did not change in a month therefore it is not a cyst of ovulation. The possibilities are listed in the report. I have pasted them below but separated the words to look at it more easily:

endometriosis
hemorrhagic cyst.
dermoid
a cystic malignancy cannot be excluded

Another possibility is an old infection in that tube from the IUD.

all the other words are just descriptive. It is like using words to draw a picture. you could even draw a picture following the directions of those words to get a mental image of what they are seeing.

so cystic means a some fluid with a thin skin around it like a blister
solid means solid
debris means old blood inside the cyst -think of it like tea leaves at the bottom of your tea cup
bilobed means two cysts side by side
echogenicity means debris or solid stuff inside a cyst that is mostly fluid

It is appropriate to have laparoscopic surgery to remove that cyst on the left.

most things are NOT cancer at your age but you will not know 100% until it is removed.

Hi There,

I think I am more fixated on the IUD causing an infection (which I know is rare with the Mirena) than that the hormones in the Mirena (progesterone) causing cysts. I think the second possibiity is unlikely

Dr. Anne

I'd like to add something on to my last post because it has to do with the same topic. As I mentioned in my first thread about my cysts I have a Mirena IUD.

I had the Mirena put in on May 9th this year and I became extremely ill after insertion almost to the point of being hospitalized. My family doctor and OB/GYN could not find anything wrong and said that they have never heard of a patient reacting this way after having Mirena put in. I figured it was because I never used any form of BC before and my body was adjusting to the hormones and a foreign object being in there. A few weeks later the Mirena expelled itself and I still decided to give it another chance.

I read one of the side effects of Mirena is that 12% of Mirena users develop ovarian cysts yet when I told both of my OB/GYN's they said they don't believe that they're being caused by the IUD. Do you think if I remove my IUD my cysts will go away and I can avoid surgery? If I leave my IUD in will the cysts come back even after surgery? Does Mirena cause complex cysts?

Sorry I know this is a lot to ask but I have been to two different GYN's and I can't keep wasting anymore money and seem not be getting any answers.
