Hi Sarah...I don't know how helpful I can be, since I don't know much about PCOS, and I didn't have multiple cysts. But I wanted to chime in with whatever I can. I just had a very large mucinous cyst removed from my right ovary...it was at least 14 x 16cm, although it burst as they were taking it out, so I don't know the actual final size. The surgery was a laparotomy, with a bikini cut incision. My recovery has been much easier than I expected! I'm at post op day 12 and I feel really good, I'm already back in class. But the entire experience was terrifying and emotionally draining. I completely understand how scared and unsure you are.
As far as what to expect at the gyno...the first thing my gyno did was send me for a more sophisticated ultrasound. A pretty basic ultrasound of my belly showed that there was a large cyst, but they needed to know more about it and what was inside it. So I was sent for a transvaginal ultrasound, in which a probe was inserted in my vagina to take the images. That was uncomfortable, but not terrible. I would assume that your gyno will send you for something similar. I also got a CAT scan so that the dr could better see the position of the cyst in relation to my abdominal organs. It was a CAT scan with contrast, so I had to drink a barium shake (gross!) and they injected iodine into a IV (made me feel really warm below the waist, which was weird!).
The cysts you have are pretty large, so you're probably going to need surgery. But they may be able to do it laproscopically, which means a much shorter recovery. That possibility depends on what kinds of cysts they are. Like I said, I don't know much about PCOS. You say that your blood test came back high... was that the CA 125? Because mine was elevated as well, and my gyno said that was to be expected with such a large cyst. An elevated CA 125 does not necessarily indicate cancer. I did end up having a gyn oncologist take my cyst out, and that was because my cyst was a complex cyst/ borderline tumor. You should be prepared for the possibility that your gyno sends you to a gyn onc - not because you have cancer, but because gyn oncs have more experience in taking out large masses that MAY be malignant. Ovarian cancer is VERY rare at age 19. (But I know that it's impossible not to think about it! I'm 22 and I was scared out of my mind).
As to your concerns about fertility, I don't know your condition or if your ovaries are damaged or anything, but I do know that gynocologists will do absolutely EVERYTHING they can to save the ovaries of such a young woman. My ovary was stretched out from such a large growth, and it has a high chance of growing another mucinous cyst - and my surgeon still left it in. She said that even if I get another cyst on it, she'd STILL recommend leaving it in. I was under the impression that the ONLY reason they would remove my ovaries would be if the cyst was malignant and the cancer had spread everywhere in the area (which, like I said, was really really unlikely).
Anyway, I hope that some of this information helped. Good luck with everything. I know that waiting between doctors appointments is the worst, but hang in there! Let me know if I can answer anything else.
