How do you differentiate cyst pain from ruptured cyst pain from ovarian twisting?

After reading a lot it seems that each of these 3 situation can cause similar pains.  

Does anyone have a clear cut/simple description of the pain of each situation?

I'm having a lot of discomfort and pain with my 5cm cyst (do not yet know if simple or complex).  I'm wondering if I would know if/when it ruptured OR if/when there was something more serious such as twisting.

So with any intense pain should a patient always go to the hospital immediately?  My doctor said it was not necessary with ovarian rupture-he said to call the drs. office instead.

In my experience, at the point of rupture - really fast rupture - the pain is bad enough that you start going in to shock. With a slower leak I'm not so sure it would be quite that dramatic.

Often, fluid in the abdomen causes it to become kind of rigid - they call it guarding - and pressing / releasing will cause pain more intense after the release.

I have a friend who had a full torsion and she was literally on the floor, but then everybody has a different pain threshold.
