It really does sound like endometriosis is a strong possibility as the cause of your cysts. After reading both reports, it seems like a positive sign that the right cyst has not grown and the left cyst has gotten smaller. With malignancy, you would most likely see growth on both sides. Unfortunately, there is just no way to know for sure without a biopsy. It is a common finding among ovarian cancer patients for their tumor to have had benign, borderline and malignant sections in the same tumor. I do think your cyst is likely benign. But, if I were in your shoes, I would be leaning toward having it removed because it has persisted for so long. Even benign complex cysts can cause problems over time. Please do let us know how it goes with your appt.

shelly this is the last tvu.. i had done in Dec 2009
bilateral ocarian cysts areidentified once again. the right ovarian cyst appears complex vs two adjacent cysts with a combined cystic structure measuring 3.3*5.6*3.3 cm representing an interval increase in size greatest dimension of 4.2cm on 10/29/09 no color doppler blood flow is demonstrated nor nodularity internally. overall the right ovary measure 3.1*6.5*4.0 cm the left ovary is also slighty enlarged measuring 3.4*5.2*4.2 cm and contains a simple cyst internally measuring 2.8*3.8*3.3cm this is also enlarge in the interval. continued clinical correlation and u/s follow up. no free fluid is demonstrated in the cul-de-sac or either adnexal compartment. the uterus is retroverted once again and it measures 4.8*8.0*5.7 cm the endometrium remains unremarkable measuring 6.0mm the cervix harbors a nabothian cyst anteriorly once again
increasing bilateral adnexal cysts. a complex cyst is identified at the right ovary vs two adjacent cysts forming a pseudo septation at the midline
Shelly thank you so much..was a lil worried cus she called me into her office to talk about results.i said that dont sound good wen they want to talk about results...they think i have endometriosis i was on lupron for 3 months had to get off, so lets wait see wat doctor has to say...i get a lot of pelvic and lower back pain and other things but im glad at least one is smaller in size....thank u again i will let u kno how it went..
The nabothian cyst is a benign condition so you no worries there. Unless it grows large and causes bothersome symptoms, it will likely require no treatment. It is good that your left cyst has decreased in size, but now you must monitor the right side. Ugh.... Well, ovarian cancer usually doesn't decrease in size, so the left cyst seems less worrisome for malignancy. But, each new cyst has to be assessed individually. Your doctor will either suggest follow-up again for the right cyst or suggest surgery. They usually proceed with surgery when complex cysts reach 5cm and yours is very close. Do you have endometriosis? It often causes complex cysts that affect both ovaries. . Perhaps your doctor can order a MRI and give you a CA-125 to be sure there are not other signs that would suggest malignancy. Most complex masses do end up being benign, but it is best to have all info. possible before proceeding into surgery or deciding to watch and wait. Good luck and keep us posted!
