Nothing working for facial nerve pain and bone on bone knee. ...Pain. 10+

Now on fentanyl patch 50mg
Every 72 hrs.  It has been on 24 hours no relief.....        prior to patch..was .up to 40er. Oxcy 4 x. Day.plus 10er2x breakthrough also nucynta Dr 250 every 12 hrs
Tried opana no benefit....I also take serpentine 200mg 2x daily
not good oxymorphine no effect.I am very worried I have 6 months before a knee replacement because it went septic....how can I get thru with this horrible pain. It is a 10after working all week there has to be something I come home and cry.....thank you for any assistance

If Nuycenta Dr is effective for you than that's the way to go - if your physician is agreeable. I agree Lyrica is not usually the best for knee pain.

So thankful for all your insight. I feel better when I read your posts. I called my pain management dr today and she is putting me back on Nuycenta Dr 250mg...she stopped it a few weeks ago,we shall see. It is hard to get and my pharmacy should have it Wednesday. I will keep the patch for another round,so far I don't see any relief.  My goal is to keep working,but when pain takes over...well you know.  On the Lyrica I have tried it and it worked on the face pain, but when the knee came into the picture it was changed.  Many thanks again

No worries about your posting ability. Don't be intimidated - it'll come. We all had to learn when we first came to MedHelp. I sincerely appreciate your response to my questions.

Gabapentin makes more sense. I - and many of us have tried it. It did nothing for my pain. The issue with the drug is that often the dose has to be so high for it to be effective that the side effects are intolerable. There's a newer, every similar medication that appears to be more effective for most called Lyrica. I was very hesitant to take it after Gabepentin failed me so miserably - much to my surprise it has been very effective in reducing my pain. Obviously it's used in combination with an opiate. You may want to discuss Lyrica with your medication provider.

It is always very difficult to find a surgeon that will try repair another surgeon's error. Part of that reason is that good-old-boys - got-your-back atmosphere that still exists among medical practitioners. It's equally true that they fear litigation. They don't want to be a part of someones mistake - even if it is to try to repair it.

Your best bet may come from a large clinic, closely associated with an even larger teaching hospital. They seem to be a bit more fearless.

Obviously opiate tolerance is a fact - but in my opinion and my physician's opinion not to the degree that many think. It's just as likely, maybe even more likely than building a tolerance is the large chance that your condition may have deteriorated. I've been on the same opiate for well over ten years. When I think it may not be as effective as it once was - all I have to do is skip a dose. I than realize that it is effective - and it's true that my condition has worsened over the years.

Adding Lyrica may be all you require - or other supportive non-opiate drugs. Unfortunately it may also be true that your dose needs to be increased. Ask your physician to think out of the box a bit and try additional medications.

All that said, 50mcgs of Fentanyl Patch is not a high dose. Those Patches are also available in 12.5mcgs - adding that to your 50mcg may better control the pain. Do you have something for break through pain? If not I'd also discuss the possibility with your physician. There are several options that should be considered to provide you better pain management.  

Knee pain is horrid. "Bad knees" runs rampant in the females of my family  for generations. Each of us either have had or will have to have knee surgery, most in the form of TKRs. I get it! My heart goes out to you.

Septicemia can occur even under the best of conditions. Sadly it's a risk associated with all surgeries - or invasive procedures.

Hello and Welcome to our Pain Management Forum. I am glad that you found us and took the time to post - even though you are in so much pain. My heart goes out to you.

How long have you been on the Fentanyl Patch? It takes 48 hours before you can really begin to feel it's effect in some instances. 50mcg. Fentanyl is approximately 80 to 100 times more potent than morphine. It's called  transdermal absorption - meaning it's absorbed through the skin. Normally a SA (short acting) opiate is RX to cover break-through pain.

Many ppl must change the patch every 48 hrs instead of 72 hours. However being new to this medication it'll take some adjusting. 50mcg is not a large dose. However it's a good starting point. You may require higher doses but due to it's potency it's best to gradually increase the dose instead of beginning at a higher one. I truly know that being patient is difficult when you are having high levels of pain.

Where have you placed the patch? Don't move it by any means - but some ppl find it's more effective in one area of their body over another. It doesn't make a lot of sense - other than you must have good circulation to the area in order to have the best absorption.

You said you are also taking Serpentine? Is that the herbal supplement also called Snake Root? It's normally used for hypertension, insomnia and anxiety. It also comes with a lot of cautions regarding interactions with medications. Could it be interfering with the effectiveness of your opiates? Compounds/medications often work differently on each of us.

It is gaberpentin. For the face pain not serpentine?spell check changed it...I am in severe pain not being to find a orthopedic surgeon to take on someone's mistake back in june.I am concerned a I seem to have built up a tolerance to pain med and worried about my future surgery for new knee.  I so appreciative of your understanding
