It looks like a muscular strain on you shoulder blade. I think it will be improved by acupuncture. It may involve with you catch cold and also you have a long term coldness inside. I guess the warm pad will make it better. I use treat this pain using point SI 19 (an acupoint). Only one point, the pain was relieved right after needle. Welcome to the Pain Management Forum. I am sorry to hear about the pain in your shoulder.
It's difficult to guess what can be the actual cause of your pain. It sounds like it may be muscle related. Sitting in one spot too long and performing repetitive motions can cause your muscle to tighten and spasm...thus producing pain....when you return to that position and notion the muscle is so fatigued that it can immediately tighten and spasm again.
Obviously you could also have a spinal or shoulder structural issue that is causing the pain. Only a consult with your PCP can provide you with an accurate answer. You may benefit from some physical therapy or your PCP may deem imaging more appropriate.
Have you tried rotating heat and ice to the area? Getting up as often as possible and changing your position may be beneficial. If you have a close co-worker a bit of massage to the area during the day may also ease the discomfort. I had a portable massage pad that I placed in my chair. It helped with muscle spasms and fatigue.
