Hi - I think it would be in your best interest to call the clinic and ask the RN for some specifics like what she wants you to chart in this journal.  For example, on a daily journal they may want an hourly entry of your level of pain (1-10), what meds you took and at what time you took them, when you felt the pain subside, etc.  You could even make a short journal entry about how you are feeling emotionally, physically, mentally.  Since it looks like you are probably in a pain management clinic it would be wise of you to call and ask them exactly what they want you to journal as this shows you are being compliant with the program.

I hope this helps!
