grinding noise and pain in right shoulder.

I have constant pain in my right shoulder and lower right back. Whenever I rotate my shoulder backward there's a grinding sound. When I stretch my right leg it's a lot tighter than my left. So I don't know if it's a muscle problem or a bone problem or even a nerve problem because the pain radiates up and down my entire right side, from my shoulder to to my lower back and down the side of my leg.

Also, I feel like I have to crack my lower back and when I do it feels so much better. I've been having this pain since about 2002.
