As Phil said it's up to the physician if he/she will require you to sign a Pain Contract. They vary.
Both drugs are lower scheduled opiates - in fact Tramadol was just recently placed in the Scheduled Drug category. It was unscheduled for years.

The use of controlled substance agreements are policy decisions made by individual physician's office. No medication prescription requires an agreement.
Both tramadol and codeine are controlled substances in the US, so I imagine that most doctors will require that you sign a such an agreement.
As far as I know Tramadol does not require a contract and most family Drs will prescribe it. I'm not sure about Tylenol though.
