Preschool Transition

I have been thinking about the continuity of preschool transition. I had some research, saying that the continuity transition defines as the two environments (school and home) are similar or compatible, and nursery/school should have some environment adjustment to make children feel comfortable and creates a smooth transition from home to nursery. But how does the nursery adjusts the enviornment everytime when the new child comes to the nursery? Especially in UK, child/ren comes to the nursery whenever the parents need? And how the nursery knows my child's need and adjust the environment? And how to assess my child is ready to school? ready for transition?
