early homograft failure

My child is 6 with Noonan Syndrome, born with patial AV canal defect and pulmonary stenosis.  He has had both is mitral and pulmonary valves replaced at age 4 yrs. 10 mos..  The mitral is doing well with a St. Jude's mechanical but the homograft failed within a year.  I have 2 questions:
1. how common is early homograft failure in children?
2. once there has been homograft failure, what is the typical protocol for replacement?
