Without evaluating her, I cannot say what is happening with her at this point. She is now essentially one year older than the discharge evaluation that you post, so I would say that she definitely requires another evaluation of her heart, at minimum, although she may need much more than that. Her iniital evaluation demonstrated an atrial septal defect, or a hole between the upper two chambers of the heart, and valvar pulmonary stenosis. These alone should not have caused a problem if nothing has changed. However, pulmonary stenosis can certainly worsen over time. As well, if these are consanguineous parents, other genetic disorders may be present that is causing a problem. Unfortunately, I am not familiar with the care in India, so cannot recommend a provider. At minimum, you need a pediatric cardiologist to see her.
