This appears to be an echocardiogram report on a 4 day old baby girl with a 5.6 mm (moderate sized) perimembranous VSD.  The report suggests an outpatient follow up with a doctor (? a pediatric cardiologist) in 4 weeks time.  This would be the appropriate thing to do.  You should read the watch lists on this website for VSD (ventricular septal defect) as this is a commonly seen congenital heart problem.  Initially, observation is all that is required.  Medication is indicated for babies who are not growing or feeding well.  Only a relatively small number need surgery to repair the VSD, but the cardiologist will review those indications with you.
