13 month with constant runny nose/swollen lymph nodes

I have a 13 month old who has had a runny nose for the past 12 months.  When he came home from the hospital he seemed to be congested and at 26 days old he was put in the hospital due to a fever and at that age they immediatly check 4 meninjitis test came back neg. .  He seems to be at his peds. every month for his runny nose he doesn't suffer from ear infections his lungs are always clear and we are told he's fine and is to young for allergies.  Well last week we went back to the Dr he had been running a low grade fever, runny nose and pulling at his ear.  Everything checked out fine and I was told that we could give him Zyrtec 1/4 tsp at night but needed to watch how he reacted to taking the med. well how can we watch how he reacts to it if he is suppose to get it b4 he goes to bed? Needless to say we havent started it yet because over the weekend he came down with a stomach virus and of course was back at his  peds on Mon.  On top of all this he has swollen lymph nodes on both sides of his neck he has had the one on the right side since he was 3 months old and the one on left since about 6 or 7 months old, keep being told it's fine and they will go away when we took him in at 3 months for this we where told that it would go away in a few weeks and not to be surprised if he got one on the other side within a week or 2 but they havent gone away and now we are considering having blood work done to check his white blood count.  I know this is long but looking for some answers and seeing if anyone else has exp. any of this. Others amy indeed have more input for you, I wouldnt give the Zyrtec.. jus my opinion, has he older siblings bringing in the regualr germs from school this is what normally happens ...If he has an infection then it would affect his white cell count but certainly if your Doctor advises this get it done .The pulling at his ear could indicate an ear in fection, what is his appetite like ?
