My son has had episodes of recurrent, intermittent vomiting going back 5-6 months. He will vomit 1-3 times a day for 3-7 days in a row, and then be totally fine for weeks, until the cycle repeats. When he is ill, he is a little less energetic, a little less hungry, but still poops and pees well, still happy, no change in level of conciousness, and no pain.
We consulted with a GI Dr, who drew some blood in early December. I got a call on Wednesday saying that my son's ALT was 495, his AST was 67, GGT slightly elevated, and WBC slightly elevated. IgE was 2000, and all other numbers were normal. INR was 1.0.
Dr wanted blood repeated, so we had that done Thursday morning. We got a call Thursday afternoon saying all that was back was INR, and that it was 1.9 and to come to the ER right away.
At the ER, they took his blood again, and ALT was 410, AST 81. INR was now 1.4, and WBCs elevated. Albumin was normal, GGT normal.
They gave my son 5mg of vitamin K, and kept him overnight. In morning, INR was down to 1.0, and an abdominal ultrasound was totally normal, chest and abdominal x-ray normal. Bilirubbin was normal in all tests.
Since my son's INR responded to the vitamin K, he was sent home on oral vitamin K with a follow-up this week.
I am so scared. What could cause elevated ALT and AST in a child? What could cause elevated INR?
How did his INR go from 1.9 at 8 AM to 1.4 at 8 PM with no medication or intervention, but then only from 1.4 to 1.0 when given vitamin K?
I am so scared this means my son has liver failure and that he will need a transplant or die. We lost his twin in utero due to my having Factor V Leiden and his cord being clotted up.
My son has no jaundice, and other then eczema and allergic rhinitis (cats and feathers) and his occasional vomiting, he seems healthy. How worried should I be? I have a history of anxiety, so I feel tortured right now, thinking I will lose him. :(
