High TSH and High T$


Daughter who is currently 11 months old, diagnosed as a Failure to Thrive baby. Has had feeding problems since about 2 months old. Sent her to a GI after forcing the issue with the Ped. that she wasn't feeding well. At 6 months old GI workup came back negative, except for reflux. At 7.5 months old my daughter's bloodwork for her thyroid came back with a high TSH 9.08 and a high T4 18.8. Her pediatrician suggested that she see a Pediatric Endocrinologist. So we took her to see one, and this is what he said. That these are unusual numbers and that you usually have one or the other but not both elavated. He wanted to run the screens again, so we did. On the 2nd run her numbers were starting to come down but he said that it looked like her body was using extra T4 instead of storing it.(not sure what the indication could be to this). He said that he wanted to consult with another Dr. on this. The numbers were: T-4 12.8H, Free T4 3.5, T3 Reverse 50(H), T3, Total 205, T-3 Uptake 27, T-4 Free 1.2, TSH 5.79, TBG 26.3. After several calls back to him, I didn't get a call back but a letter that said after reviewing everything with the other Dr. there is no evidence of thyroid disease, and that she seems to be recovering from an unknown influence on thyroid status or hormone binding. Recommended a re-run of the test in 3 months. I just took her to a Genetics Doctor for some testing because she doesn't seem to be growing well. I told her about the thyroid issue and she said well we can run the screens again for you to see if there is a change. Sure enough her TSH is high again 8.62, T3 total is 137. Waiting on the T4 results yet. She was 10.5 moths old when these last screens were done. I'm very confused about this situation. I'm terribly worried about her developmentally long term wise. So far she has been meeting all of her milestones at her current age. The one thing I notice different about her is that she seems at times to not have alot of pep, lack of energy.  I'm concerned that the Endo Doc. is missing something and I'm not sure where to tell the next doctor what to look for because I'm getting a 2nd opinion. Is it normal for her TSH to be going up and down? What would cause her TSH and T4 to be elevated? My other concern is if this has been going on since she was about a month old. Her newborn screens were all normal. Could she have developed some type of thyroid problem shortly after birth?

It is odd that both of her levels are high. I, personally, have never heard of it myself. I'm very glad you're getting a second opinion, and if need be get a third opinion or more. The levels shouldn't rise and fall. If they have been elevated they can naturally go down after a while, but should rise again. Some problems can be missed on the newborn screens, as the amounts haven't risen quickly enough to be caught on those tests. Good luck!
