My 2 year old has a painful & itchy penis

This January my son (2 years old) got a rash in the pelvic area that became infected, having a white top on each spot. He was prescribed mupirocin and he got well. The rash came back, also in his legs (like spreading) Everytime he has the rash (looks like pimples 5-6 at the time in the pelvic area and his penis very swallen and red) The rash came back for the 3rd time and the doctor said that was yeast, now he was prescribed nystatin, got better. Now for the 4th time, it came back, I put immediatly the mupirocin, but this time the rash persisted and I was told to put also the nystatin, the rash is going away, but not the redness in the penis and he screams everytime we change his diaper and complains about pain all day and all night. I don't think this is a yeast infection anymore, i feel he has a virus in his system or maybe something related to the penis that makes him scream. please help me?

He could be allergic to the soap or whatever you use to clean him with some children are sensitive to wet wipes, use nothing but water also when you bathe him no shower stuff or bubble baths,also the creams put on him to soothe it ,  check out the diapers he wears , perhaps they have some made of hypoallergenic material. they would be better.

Yeah! he might be allergic to the soap or shampoo you're using I kinda had the same problem with my 4 year old girl when she was 1 year old,hers was rash around the outside of her vagina... I try  Aquaphor Baby is a body wash unscented, and Aquaphor diaper rash since I used  their Healing oitment for dry skin from Eucerin other brand made from them too.
