Penis Infection

Me and my wife recently had our nephew over. I was aware of him getting an infection in the the penis a while ago. the reason being is because he is not circumsise his mom told me that she took him to the doctor and he said that the skin needs to be pulled back all the way and cleaned every day. Im pretty familiar with this because i had this problem when i was a child as well. Well this is the problem my wife and me was checking out his penis i pulled the skin back and i noticed that i couldent pull any futher also i could not see the cone shape tip of the penis where it looks like a mushrom. i was not sure if the skin couldent go back more or if it was., since i didnt see the full head out it kind off look like it was all an even surface. I did not see any plaque or any sign of infection. Is it normal for a child not to have a cone shape tip of the penis? Because i never seen a penis like that in my life. Or maybe i didint pull back on the skin any further. Well i dont know what to do and im pretty worried about him what should i do? He is about 4 years old.
