Son has one hard, red swollen nipple

My son is 14 mo old and his right nipple has a red circle around it and appears to be slightly swollen. It feels hard behind the nipple as well. His left nipple is perfectly normal. I noticed this a month or so ago, but it went away so I just thought he scratched it on something. His nipple has been red for the past 24 hours. Do you know what this could be caused from?

My 19 mo old has the same thing with his left nipple. The pediatrician said he'd never seen that before and rx'd antibiotics in case it was a bacterial infection. Did your son's nipple ever resolve?
