My 22 month grandson called for help (I was there in 30 seconds).  He was hanging on the edge of his toy box and it appeared that he was semi-conscious by the time I grabbed him and he was limp.  His color was good but he seem drowsy for about 15 seconds then it was back to trying to empty his toy box.  Could he have started falling asleep or was he passing out?  I called my daughter (paramedic) and she said she never heard of anything like that.

It's important to consider hypoglycemia.  If your grandson hadn't eaten or had sufficient protein, he could have been having a hypoglycemic attack.  While frightening, they typically are not dangerous in and of themselves as long as you feed them protein quickly.  The major concern about hypoglycemia is it is a precursor to diabetes.  However, at that age, I wouldn't be as concerned about that as just making sure he eats protein regularly - almonds are a great source of that, as well as dark trukey meat.  Even a bit of cheese should help.
