bowel probelms with 4 year old

my daughter is now 4 and still has problems with her bowels being constipated all the time , she hads been on movicol for 9 months now and also refuses to pass her bowels on the toilet instead she poos in her pants but does not say anything. when the movicol is stopped for 2 days she becomes constipated again. is this possibly behavoural or something more serious. please help
