cardiac tamponade secondary to long line migration

is it possible to spot this sooner as my daughter had a fast heart rate for a nnmber of hours before but the doctors didnt thinknanything was wrong. she was having a cardiac tamponade secondary to long line migration,we nearly lost her she had no out put for nearly 5 mins now she is severly braindamaged has cerebral palsy seziures and is blind .on the mda website i read up on the warning signs for a cardiac tamponade and a fast herat rate is a warning sign .but it was left untreated right up untill the last moment and was nearly to late can you advise me if this alone should have alerted the team in the intensive special baby care unit earlyer and if so could have been pevented or treated sooner and she possibly wouldnt be suffering so much now i would really value your opinion from what i have read on the mda website i believe this could have been prevented or treated before it became life threating
