my sister watches a child....

my sister watches a child, a little girl, and today she developed a rash on her stomach and with the rash are a few bumps. the child is 2 years old. i told her to get ahold of one of the parents and take her to the doctor, they refused. "if it starts to spread then i'll take her". is what the father said. well it has started to spread and gotten slightly worse. she didnt have it the day before just today. i have a small history in health care and ive seen hundreds of rashes on adults and children, but ive never seen anything like this... i told her it could be a number of things. she said it doesnt seem to be itchy or pimply, its just a large red patch on the stomach, with a few bumps on her stomach and two on her back. i dont know what to tell her to do...
any advice..?

i would refuse to watch her unless it is taken care of--if she has other children around, she could pass what ever it is to them--not to mention, lots of things start small and can turn into anything--what if something major becomes of it while she is in her care?
