I had similar questions after I was asdvised that I had PCOS due to insulin resistance and weight gain. I was prescibed Metformin and informed to follow a diet that practically eliminated carbohydrates. I am also TTC and have two children but it has been over a year TTC. Cannella gave good info and I also advise to follow up with a doctor and have hormone test completed.

ask for a refferal to endocronologist if you can they probbaly will do the hormone tests and if they think you have it most probably will prescribe with metformin which helps conceiving in most cases , also losing even small ammount of weight will improve fertility if you have PCOS ,
just weight gain and heavy periods itself shoulndt indicate PCOS , but if you have irregular periods it may be sign of some hormonal imbalance . and endocrinologist should be able to pinpoint the reason you cannot conceive. PCOS is the most common fertility problem but not the only one , like high levels of prolactine can cause infertility aswell which also checked with the series of hormonal tests to diagnose PCOS , and if they cannot find anythign hormonal they may need to check your reproductive organs with ultrasound to see if there are any blockages in tubes etc.
but either way dont worry , since you already been pregnant before it means you most probably can again , just need to pin point the obstacle infront of it . best of luck :)
Blood work she would check for hormonal imbalance (FSH,TSH,LH,Prolactin). The pill will help regulate your cycle but that's totally up to you.
Your doctor will be your best bet to know and the bloodwork will confirm her diagnosis. I know with PCOS that weight, facial hair, irregular/non-existent periods are more of the common symptoms and more obvious.
