Tubes are not 'tied' in the sense of knots. They either use clips, or they clip and burn the ends. All methods have less than a 1% chance of failure, with the clips being more likely.
A reversal can be done, but this depends on what was done, amount of damage to the tubes and the skill of the doctor. Be warned, it is costly (ranging anywhere from $5,000-$10,000) and is not a guarantee of working.
Yes you can. It happened to one of my instructors. Your body can grow another tube around the ones that were tied and burned. So yea it is possible, I don't know how common it is though.
