well, right before you nurse, get some towels soaked in hot water, and do that for 15 mins, and then nurse, the swelling and pain are just from your milk being in and ready to go.  The steam and warmth will help release.  Sometimes I had to jump in the shower afterwards to let the milk drain, or pump to start saving up on milk
