Back Fusion and Giving Birth

I am pregnant with my first child. I had a back fusion L-4 to L-5, 9 years ago. I have an extra vertebra in my back. One back doctor said I might need to have a C-section due to my pelvis being titled. I asked my nurse midwife if I would need a C-Section and she said "we will wait and see when the baby arrives. When I am giving birth, if it doesn't come out, we will do a C-section".
Should I be concerned? Can my fusion break under pressure? Can I have a C-Section last minute?
