No Foetal pole and Yolk Sac seen

Hi Doc,

My wife,Who got positive during her pregnancy check using kit. We went to gynac and she asked for vaginal u/s and as per the u/s result she recommend some medicine and full bed rest for two week period. At this moment i am not sure what is this all about as per the u/s report its writen.... Uterus is bulky and shows presence of well defined gestational sac. (1.5 cm) corresponding to 4wk and 5d (~1 wk).Poor decidual reaction is noted arround sac. No Foetal pole and yolk sac seen. Bilateral ovaries normal in size. No fluid in cul-de-sac. Repeat u/s in 1-2 wk.
What does it really means..
1. Is she is pregnant with some risk or is this normal?
2. What should be the next step?

wait the 1-2 weeks. She may be earlier than was expected. At The next scan you may see a baby.

I would like to mention that my wife not feeling any pregnency symptoms as well, Doctor also prescribed some medicine...1. Duphaston 2. Proluton Depot medicine.

When I had an ultrasound at 5 week they didn't see a yolk sac, actually just told me I was pregnant but nothing was too visible at that time. I would like to report that I have a healthy baby with a strong heartbeat.
