PSA Doubling time

My husband sadly died of protate cancer with bone metastases this year He went to GP in Feb 2111 with urinary problems At that time he had PSA of 5. and a rectal examination showed slight enlargement
He was told he was fine

By 2113 after a fall and  Xrays showed extensive metastases and PSA of 10.2

I am told in all probability he had mestastases in Feb 2111 according to doubling time

I dont know how they worked that out

   Can any one help

In April 2113 his Alkaline Phosphatase was raised to 2388 but back in 28 Dec 2111 it was 93 within normal range I am convinced if the doctors had refered him  at that time  he couuld have had prostate removed

They say different that he already had metastases How can they know that!
