Mysterious urination, ejaculation pain and infections symptoms

If you can figure this one out you are a genius.  I have constant severe pain when urinating, during and after ejaculation, and have painful stinging and itching in the end of my penis at all times.  Frequent urination and occasional slight leaking.  I've been dealing with this since 2002. I went to several urologists and did the usual long term antibiotics, Flomax etc.  I don't drink alchohol.  Had swabs put in the ned of my penis to check for infrctions, all is clean.  Had a scope put in the end of my penis and checked my bladder which was fine.  No IC or bladder cancer.  Had Cat-scan done of pelvic area all was fine.  So I thought it might be my back.  Had surgery in '87 and had two discs removed L-4 and L-5, with nothing put back in and space is now filled with arthritis.  Also vertebrae were not fused. A back guru thinks the unstable vertebrae may be the problem, if so, why the constant stinging and itching like an infection? I've had Pudendal and caudal facet blocks on both sides with no relief.  When I was younger, late 20's and all of my 30's I had frequent Chlamydia and because I drank alchohol excessively, antibiotics never really worked.  Quit drinking in 2001 at age 43.  I had extensive dental implants done recently and was on Metronadazole for 6 weeks to fight bone infection.  Still have raging infection-like symptoms in my penis.  Help!!!!
