high ASO Titers

My son who is 12 years old has had trouble with strep in the past.  We were on our way to having his ASO's to below 400.  He has not had strep in at least 6 months and now they are back up to 749.  This worries us because he already  has a mild heart murmer from the ASO's being high before.  We are going to be taking his to St. Jude, to the infectious disease center, in the next week or so.  So my question is, What should I asked them, and what should I know before going there.  Also how much damage is this doing to my childs heart and kidneys?  Does anyone have a clue to this????
