Advir to treat pneumonia

My Dad was an active 75 year old.  He had a history of heart attacks and had a pacemaker.  He came down with a cold which turned into pneumonia.  He was hospitalized and immediately put on Advair.  Within two weeks, he was dead from "pulmonary fibrosis, unknown etiology."  I have read that Advair can cause pneumonia, especially in people over 65.  My question is, is there any reason why he would have been put on Advair when he had pneumonia?  He was not a smoker and had no history of lung problems.
