Chronic Cough for 5 years

I started having this cough about 1 year after moving to Indianapolis, IN (moved from northwest Indiana, and moved back up last August). It is several large hard coughs for a couple minutes and continues throughout the day, with lots of thick mucous/snot coming out of my throat. It is the worst first thing in the morning, bad enough to where I normally end up throwing up stomach acid from how hard I am coughing.
I have been tested for TB, have had a specimin test for what I am spitting up, been given a chest x-ray, have been giving an inhalor for ashma for a couple months (doubled my dosage after no change the first month, still no change the second month), given something for acid reflex, and have tried mucinex (trying again now), and have been given a breathing test (I was in the 80's percentile). I think that's it.
I work as an automotive technician, but when this cough started I was in college and worked at a grocery store. I recently lost my health insurance, had it free through my work but they took that away. So, any help would be appreciated. It has progressed some in the 5 years. I thow up in the morning regularly now, due to the intense coughing, started 4-5 years ago but more regularly now. It occurs whether I have eaten or not, and it is bad enough that I tend to skip breakfast in the morning so that it is only stomach acid coming up. If I have sex I will usually start coughing really hard to where I also thow up, this has been occuring regularly for 4-5 years.
Lately I have had periods lasting a about a week where my throat is so congested or clogged to where it is hard to breath and hurts to swallow(really bad when I first get up), it feels better after coughing and spitting up for 30 + min. Around the time this started I noticed that I had hernia and had that fixed, it was that doctor that prescribed me something for allergies and then acid reflex.
I don't believe that it is contagous, because I have lived with my girlfriend (same one) since moving to Indy (over 6 years). She has never got this, I worked at the grocery store for 2 years (no co-workers got it), and worked at a chevy dealer for 3 years (no co-workers got it there either). Now I have a daughter, 6 months old, and she doesn't have it and neither have any relatives.
If you have any questions please ask, I will check this regularly. By the way I am a 25 year old male, 6' - 6'1", and weigh 245 lbs. This started when I was 19-20.

Indianapolis is not that bad.  It's possibly, maybe likely, a metabolism problem, related to insulin, and some resultant endocrine problems.  Read "The Program" by Dr. Diana Schwarzbein, to solve these problems.  Also recommend The Ultramind Solution by Dr. Mark Hyman, but it's a lot more science to wade through.
