This may be totally off the wall but have they looked at your gall bladder? It is worth a shot to look at if the docs have cleared you for cardiac and pulmonary problems. Gall bladder pain does give a sharp pain in the shoulder blades sometimes along with that full sensation. An ultrasound would be best for looking for stones in the gallbladder, it is easy and painless to do.
