Heavy Breathing

I have been suffering with heavy breathing from quite some time. I have cough also. I feel short of breath even if I climb around 10 stairs. Have too take regular deep breaths during the day in span of every 5 mins to keep myself comfortable. I even feel that most of the time my left side Nasal is blocked and i am abl to breathe only from right side nasal. I even keep getting mild chest pains during the day. At night most of the time I have breathe from my mouth.
Can somebody suggest me what I should do, or what could be the cause of these symptoms.
