A musculoskeletal cause of such pain is likely in a healthy, physically active 23-year-old, especially when the chest x-ray and EKG are truly normal.  The radiation of the pain to your shoulder and your perception of the pain as being in the muscle are consistent with muscle strain.  However this could also be due to stretched or compressed nerves in that region of your body.  This is sometimes referred to as plexopathies, which is pain, weakness, and muscle atrophy, originating in a network of nerves.  One such plexus is the brachial plexus in the armpit area.

You should check with your doctor about this possibility.  If the pain continues, consult with a neurologist.

hi james, I am a runner as well.  I am 5'4" and usually 110lbs ( I am pregnant right at the moment) and I experience the same pain occasionally.  I had been to see my dr and had an ekg as well.  I was diagnosed w/ muscle strain.  My dr says probably from pumping my arms and i am not an extrem arm pumper.  It never has lasted form more than a week at a time so I never really worried about it.  I took warm baths and relaxed after long runs and found my self really relaxing. I also incorporate more yoga into my daily routines and stretching has really helped w/ all my muscle woes.  Of course if nothing you do on your own helps pursue w/ 2nd opinion from a dr.  good luck

I know this is going to sound weird, and it may not be what is happening to you, but many years ago a friend of mine had similar attacks.  The only reason I really remeber this was because it was in a real weird location.  We use to say she was having a "tit attack", but it turned out to be her stomach having an attack.

Here is my theroy on this:  Our stomach is located right below the left breast. I guess hunger, gas, upset stomach can cause pain.  Due to the location it seems like it is in the lower chest area, but its actually the stomach.

I personally haven't had this problem but since then I have known several people who have experianced similar attacks and a piece of candy or eating something took the pain right away.

Good luck!
