Lung cancer, emphysema and vomiting.

My father's got a computed tomography done, he is 85, the report says:  pleural derrame in right lung with two  sub pleural polylobulated nodular images, one of them measuring 20mm of diameter.
Emphysema with fibrotic emphysematous component that distorts the lung architecture; bronquioectasic zones decreasing the right lung volume by atelectasia + hyperemia due to hypoventilation.
Hilius are somehow enlarged with vascular components.
At mediastine level: Several adenomegalias, some of them pre carinal that pushes Superior vena cava. Some adenomegalia 37 by 35 mm present central necrosis, some are smaller measuring 1 cm and located in lung aorta. Another adenomegalia subcarinal located in posterior mediastine measuring 34 by 26 mm.
Bronchial trachea with good definition. Esophagous displaced by the subcarinal adenomegalia.
My father has no symptoms except for nause and vomiting, which is the reason why he has been hospitalized for 5 days. I do not know what medications he is taking. He has lost weight -prior to this episode- 4 to 5 kilos total.
I do not know the diagnostic. I would like to know what do you think this report shows and the prognosis. I have not talked to his doctor yet but I understand that all the elements on this tomography relate to lung cancer. I believe the doctor wants to perform a biopsy or a fine needle aspiration.

The report shows much lymph node enlargement.  This is often seen with lung cancer.  Your father's doctors will need to carefully examine both his lungs and his upper gastrointestinal system.  I would agree that he would probably need a biopsy or needle aspiration.  If this testing proves that he has inoperable cancer, you should ask his doctors to carefully explain all the options to you and your father.  Be aware that sometimes the best option is to do nothing but provide pain relief and relief of other symptoms, such as his nausea and vomiting.
