Pneumonia, TB, Stroke...

Help, My father-in-law (67Years old) had a stroke 3 weeks ago (Right sided paralysis, no or little cough/speech ability) He had a crainectomy to relieve pressure on the brain. I understand it is common for stroke patients to get pneumonia but he is getting very sick fast.  He has a Tracheostomy and a PEG.  Tests showed that he also has Tuberculosis that may have been there for years.  Just today he BP went low and his fever is increasing, they have done some tests on his heart and found that he has heart valve problems and it also seems infection has spread to his liver and kidneys.  Can anyone give me any hope that he is going to be ok. I am assuming that the infection has spread from the pneumonia, is this common, is this a very bad sign or is it still possible for recovery? He has been on Antibiotics for his pneumonia for over 4 days now, shouldnt it be getting better not worse? Should the medicine be getting changed? What should we be asking the doctors? He has made it this far and we are NOT giving up on hope. please can I hear from anyone. Thanks. (I also have posts in Neurology and Stroke forums)
