Sinus Infection ?

Hey doc, I am not sure how to put this but here. I have had my sinus infection for about 9-15 months now. It gets better, but only for a day or two and it's back. Sometimes only nose is clogged, some days ears, nose, and all other sinus cavities. I know I'm not the healthiest man on earth, but I can't take it no more. My doctor had given me from allergy meds to antibiotics. The only time it clears up is when I'm isolated in my house, meaning i don't leave it at all for a few days. So what could this be? - Allergy, inflammation, a literal clog in the sinus ? I don't know, but I would love to know. Thank you.
