chest or back pain

I have this pain it seems to be in my upper back area, but when i take a breath (not even a deep one) burp, or laugh i get this really sharp pain. I can even laugh or move my right arm in the wrong way or the pain comes. When i eat or even swallow my slava i can feel it going down and once it gets to this certain spot it hurts really bad. I am not able to sleep on my back, sleeping period is very uncomfortable & painful. Can anyone tell me what this could be.
