lung granuloma 5"

I had a ct scan early Aug. 2009 an a granuloma was seen in both lungs. The one in my right lower lung was aprox 5". I know that there was nothing pressent as of Jan.2008 due to another ct scan and x ray done then.
I have occasional chest pain and shortness of breath both with exsertion as well as when I lay down to sleep.
Should I be worried?
