percardial effusion with lung nodules

I had a Ct Scan and it showed percardial effusion and the last time I had it done I only had 1 nodule,now I have 4 nodules in various areas of the right lobe. I have Lupus and Scleroderma with alot of complications...my lung dr. said it could be from an old infection?!!  can you tell is that really true...confused and concerned . And,what do they do for the effusion an Echo also confirmed the effusion too.  Can you please answer. I asked this question in Dec. thanks
