pulmonary nodules pericardial effusion

I recently had a ct scan of my chest and it  showed two 2mm pulmonary nodules within the right lower lobe,also there is a 2mm pleural based nodule within the right lower lobe a 3mm nodule is also noted within the right apex anteriorly. And there is a small pericardial effusion seen. Everything else is unremarkable. Except there is mild degenerative changes in the spine.
I do have Lupus with Scleroderma-with alot of complications is this something to worry about? my last ct scan just showed 1 tiny nodule..My one Dr.said it could be from previous infections that I had,or that as you get older your lungs shrink??? that just doesn't sound  right --
So,could you please give me your opinion of what could be going on and what should be done next--
