re-current sinus infections with Abx/steroids for year!!

by lalove, Jun 23, 2009 10:03PM
Hi,

I'm hoping you can help me!  I'm writing in regards to my husband who has been suffering from sinusitis for almost a year (it tends to occur secondary to colds/or exercise-induced asthma).  He has never presented with any drainage, congestion, or fever. His only symptom is "pressure" that is primarily mid-forehead & over eyes. He has been on numerous antibiotics with steroids beginning October 2008; about 8 courses of antibiotics and medrol dose packs/prednisone. In order, he took Amoxicillan, z-pack, Bactrim, ceftin, cefadroxil, and now Augmentin.  He does not tolerate leavquin or other abx in that class. He has seen an ENT & CT scan showed nothing too significant (maybe some swelling- couldn't discern if inflammation or infection).  With bactrim and Augmentin, he got some relief after taking long courses- but never felt 100%.  Prednisone only seems to help for a day or two- and when he weens off, he feels much worse again.  Outside of conventional medicine, he has tried sinus irrigation daily, allergy meds (including singulair, claritin, flonase, nasonex, allegra, ect).  He does not think this is allergy-related as he never gets congestion, sneezing, post-nasal drip, ect.  We have also tried alternative remedies such as oil of oregeno, bromelin with quercitin, garlic, apple-cider vinegar, ect..

He feels so hopeless at this time that he has left the state too deduct any environmental allergies.  I am also very frustrated what to do next.  We don't want to continue with antibiotics & steroids as we are concerned about the safety and negative consequences of long-term use.   We also don't want this to get worse & cause complications such as meningitis.

My main questions at this time are:

1- Can chronic (bacterial) sinus infections ever get better without taking antibiotic/steroids?
2- Do some steroids work better for some people than others- (ex- Prednisone doesn't seem to work- would if be beneficial to try dexamethasone)?
3- Can saline irrigation cause increase inflammation- could it har rather than help)
4- Any opinion on using hydrogen peroxide mix with saline irrigations?
5- What is likelihood of anerobic, MRSA, or fungal infections?
6- Any benefit in obtaining 2nd CT scan?

Please feel free to respond to some or all of questions!  I would just appreciate any help or feedback!!!
Thanks
