what causes my chest pain and back pain during minimal breathing?

i always have a prodrome of central chest tightness sort of pain radiating to throat before discovery of blood coughed up. i also have upper back / right shoulder blade pain together with the chest discomfort and also RUQ pain. i feel these area tightness and hard when breathing in.
My question is:
1.what causes pain in the chest and upper back in terms of appearance of hemoptysis. eg ?cancer ?where ?airways ?esophagus
2. my tightness continues day and night, breathing in is a problem already, expanding chest a problem, seems that my sputum cannot be effectively coughed up. how to solve?
3. since i always try to cough up the 'mass/discomfort' in the middle of chest, i reallly use up all my effort to cough, what sort of complication will chronic cough may cause?
4. can CXR rule out cancer or obstruction in the airways when i have the above progressive symptoms.
5. i feel that my breathing effort is weakened esp during sleep because i suddenly wake up to learn that i cannot breathe on my own and the breathing seems to stop by itself. it complicates me with palpitation which make me feel that my heart will stop. what can cause this?
6. have i got sinister disease? is it in advanced stage? oh........  :(

Chest pain can originate in the lungs or the heart.  You must have a thorough evaluation to find out what is causing your symptoms.  A CT scan is very sensitive in detecting the cause of hemoptysis.  Coughed-up blood usually comes from the respiratory tract, rather than from the esophagus or other part of the digestive tract.  You need a repeat CT scan and possibly a bronchoscopy.  This is a procedure where a tube called a bronchoscope is passed through your nose into your windpipe to look into your lungs.  A pulmonologist would be the type of specialist to identify if you have a lung problem and the best treatment.  A cardiologist is the type of specialist to identify if you have a heart problem and the best treatment.

Your symptoms also suggest that you may have a sleep disorder, such as obstructive sleep apnea.  This is a condition in which the airway does not fully open during sleep.  This causes breathing to stop temporarily, for up to a minute at a time, during sleep.
