Over 4 Weeks and Penis Pain

HI Dr,

I'm a 26 year old male, uncircumcised heterosexual.

Exposure:
6 Wks ago I slept with a prositute, I used a condom for both protected vagina intercourse and oral.
4.5 Weeks ago I had a one night stand. I used a condom for the intercourse but she also kissed my penis (quite briefly) on the head of it unprotected.

Symptons:
The next day after the 2nd exposure, I noticed some discomfort between the tip/opening of my penis and where the head and the foreskin join the rim (the under side of the head). This discomfort has continued now for the last 4.5 wks. It is not constant, it gets worse at the end of the day and after (not during) I urinate. I have no other symptons.

Testing Done:

After 2 weeks I went to a GP who took a urine sample, swab test of the inside of my penis and a blood test and gave me 2 tablets to take incase of gonerria. All tests came back negative. Tests were for chlamydia, gonneria, herpes, HIV, syphalyss. He also inspected my penis and said there "is no visable signs of any std's"

After another week with the burning, I went to another GP and got all the tests redone. Once again they all came back negative. He also inspected my penis and said there was no signs of STDs. This doctor was my normal GP, and since he knows I tend to panic about this stuff ( I was tested less than a few months ago) he said I don't have any std's and perscribed me tablets  called "Sertraline" (well thats what is on the box) to help me stop checking my penis all the time.

Yesterday I went to a 3rd GP who inspected my penis and said no symptons.

Questions:
the fact a herpes blood test doesn't work until the antibodies are in your blood and the pain persists I am worried I have this. Does this sound like herpes? Since my test was 2 weeks after my sympton started, would antibodies be present? How common is genitle herpes with no other symptons? any other opinion? Unfortunetly I live too far away from dermatoly/sex clinics.

Thank you very much for your post and welcome to our STD forum.
It is difficult to give you an accurate diagnosis whithout actually examining you.  However I can certainly say that from the description of your symptoms, it does not seem to be herpes.  Symptoms seem to be more consistent with NSU or Non-Specific Urethritis, which is an infection of the urethra caused by bacteria that are not Chlamydia or Gonorrhoea, for which you were tested negative.  If you are unable to get a test for this, it might be worht getting an empirical treatment with antibiotics that would treat this condition to see if they clear your symptoms.  You could take Doxycycline 100mg twice daily and Metronidazole 400mg three times daily, both for one week.

Just wanted to add also that although the doctors see no other symptons, I think my entire penis head looks quite red compared to what I thought it looked like before, and when errect I can see some lumps on the top of the head, although they can kind of be seen when  the penis is flacid also (very very small) and looks like tiny dots, the doctor said this is normal.  please take this into consideration also with the above. Thankyou very much.
