Some advice please

Hi Dr

Can you please advise.  I had unprotected sex with a female (vaginal) just under 5 weeks.  Within one week my problems started, burning sensation in scrotum and tip of penis but no discharge.  PLease bew aware I had same episode last year with same woman and had exactly the same issues, I know stupid, I should have learnt.  I went to doctors who prescribed me 7 days doxycycline tablets which did not help.  The problems over the next couple of weeks have persisted with new symptoms coming along.  I have been getting burning sensations in neck, elbows, ankles and behind my knees a stiff tight feeling.  The glands do not appear to be swollen.  I visited GUM clinic who tested for Chlamydia and Gonnorea at week three but came back negative, doctor gave me 1gram of azithromycin which again did not take way my problems.  I re visited the GUM clinic who again took further swabs and tested for HIV and syphillis at four and a half weeks, both have come back negative.  The GUM clinic use antigen and anti body test for HIV,  I tested last year at 6 weeks post exposure then 13 weeks both were neg then too.  This morning I now have a sore throat but no temperature and dont feel like I am getting a cold etc...  I stress everything exactly the same as last year.  The doctor has prescribed 2 weeks course of doxycycline this time round, can you suggest what this could be all standard stds negative but this has to be related to my encounter with this lady as the same things happened last time.  Thanks in advance.

Finished course of 2 weeks doxy and still my problems persisit and getting no where fast, the pain in scrotum is milder now but still there and seems to be locally to my testicles (both, however not sore to touch), I also still have strange sensastion behind my knees, as if they feel tight but not painful.  This may sound strange but I have noticed my sperm doesnt have the same appearance, its seems a lot thicker and creamier, could this be sign of something, still getting pains under arms and occassionally in elbows.  Please give me your thoughts.

Hi Doctor,

One week in on doxy and my symtoms persist, generally feelin unwell. Pain and burning sensation in scrotum going into the thighs worse when sitting.  Would I expect by now for the symptoms to have subsided if this course of doxy was going to clear.

Hello again,
Even though not typical, your symptoms could certainly be those of NSU.  The HIV test that you had is the Duo or Combi test which can be considered conclusive after four weeks.  Therefore you can definitely exclude that.
See how you get on with the two week course of doxycycline, which hopefully will clear all the symptoms.  Otherwise you might need to have specific tests as mentioned above.

Thanks for that,  if it was a NSU would my symptoms as described above be what you would expect after only 4 and a half weeks post exposure.  How reliable is the HIV P24 antigen and antibody test at 4 and half weeks, ie how many would turn positive at 3 months, quite sure its not that as tested out to 3 months last year after similar problems after having sex with this lady.  My lymph nodes not swollen but feel what I can only describe as sharp burning pain in elbow and ankles, and tight feeling behind knees,  I also have just developed a sore throat yesterday.  I suppose just want to rule out HIV again.  Am on 2 week course of doxycycline now so hopefully we resolve.  Can you please advise on my questions above.

Thank you very much for your post and welcome to our forum.
We can certainly exclude Chlamydia, Gonorrhoea, Syphilis and HIV.  All the tests are fully conclusive.
The symptoms that you describe could be consistent with a number of sexually transmitted diseases, including NSU (Non-specific Urethritis), which can be caused by micro-organisms such Mycoplasma, Ureaplasma, Gardenerella, Trichomona and others.  The first two usually respond to the antibiotics that you have had.  However you might want to have specific PCR tests for these.  Also you might have contracted genital herpes and it might be a good idea to have both a PCR herpes swab and a serology blood test.
I hope this helps.  It is difficult to be more specific without examining you properly.
