3 month long rash that changes appearance and location often

Hi there,

I was hoping Dr H Hunter Handsfield or another doctor with STD knowledge could assist me please.

Approx 3 months ago i had unprotected sex and also used a cheap lubricant designed to delay my ejaculation and prolong my sexual intercourse. About 3-4 days later I noticed a couple of red patches on my penis head and became concerned. I had changed shower gel and picked up some fresh laundry the day before as well. I was travelling at the time and went to a free doctor but he could not be sure what it was, suggesting it could be herpes but could be many other things as well. He gave me two injections - Penicilin and Ceftriaxone and suggested I apply aciclovir to my penis as well. There was no pain and no raised bumps similar to pimples or blisters.

I did this for one week and the rash was still there. I also developed a runny nose around 6-7 days after noticing the rash but convinced myself this was due to the extreme stress and panic that consumed me during the week prior. At no stage was there any pain, and as I suffer from oral cold sores I did not see any similarities at this stage, not even any tingling or itchiness. I then went to another Doctor who said it was nothing, just an irritation from the hot weather and staying in wet board shorts and a change of shower gel etc, who then gave me another cream to use for one week. After this one week it got a lot worse. It began to peel much like sunburnt skin would and the whole penis head and foreskin became very inflamed. At this stage it was very itchy. This lasted 3-4 days at it's peak. I was still travelling so sent some pictures to my girlfriend who showed them to her doctor. That Doctor said it was a yeast/fungal infection and that I should use "Trok" cream on it for one week. This certainly helped things, but the rash is still there and has not yet cleared up. Now back in my home city, I went to see yet another doctor, who based on the story above could not be sure either, thinking it was either a yeast infection or herpes. I showed him my penis but he said there was nothing to take a swab from and that they do no like to do blood tests for this as they often get false results, therefore the only real way to confirm is to take a swab during an outbreak. However I have never had an outbreak so this kind of left me in limbo - I think I have it but can't be sure. He suggested I use the Trok cream twice a day for 2 weeks and I also had an oral dose of Flucanazole 5 days ago, with little change. Needless to say this is driving me crazy and making me feel very depressed, it's the first thing on my mind each day, the last thing at night, and often I dream about issues relating to this.

One thing that confuses me is having had many cold sores in the past, the are usually in the exact same location, or one of three places. This is consistent with what I have read about genital herpes too. However the rash on my penis changes hour to hour - one time it may be right near the tip/opening, one time it may be around to the right, one time it might be in the middle. Often there are dozens of small purple/red tiny dots if you look closely and push on the penis head a little. When I get an erection my penis head becomes very red, likewise when I masturbate it becomes very red and stays this way for a few hours. When I have shown the doctors my penis they have said they can't see much there, and yet I have had some sort of rash or redness on my penis head everyday for 3 months. Again, it appears to me that this is not consistent with genital herpes, however the thrush/fungal treatment has not be effective at this stage.

Is it possible that I was "shredding" or my oral cold sore was flaring up, I kissed my girlfriend, then she performed oral sex on me, she has given me genital herpes? She does not suffer from cold sores.

Also, how long can I use the Trok cream for? 2/3 weeks without causing any permanent damage to the penis head?

Could you please help me by providing any advice or diagnosis to the best of your ability over the internet. If you would like me to upload some photos I can, mindful that the rash and redness is not consistent from day to day.

This is a free forum, the Dr's forums are currently closed.

Skin issues can be many things from a STD like herpes to non-STD issues. Only way to know for sure is to see a dermatologist. Blood test would not be a bad idea.

Sorry - in addition - I have obviously been travelling a lot and therefore my diet has been poor, immune system low, many late nights and plenty of alcohol (mainly beer) has been consumed during these last few months.
