A few questions.

Hi,

about 5 years ago broke up with my girlfriend of the time, for about 2 months. Was assured that she had not slept with anyone else. After a few weeks of having unprotected sex, I had a burning sensation. Anyway, due to being unable to find a slot at a GUM clinic and work commitments abroad, I was unable to be checked for about 2 months or so. Test results were negative for everything. Burning sensation had reduced but not totally disappeared. Penis "felt" different though. This gradually increased until urine flow became slow.

Questions I have are:

Can Chlamydia heals by itself?(most likely I DID catch something from her).
Can it cause stricture? How likely and any particular time frame?
Can it cause anything other than stricture that could lead to reduced urine flow?
So fast forward to now:
There is often a tingling feeling within the penis. It also looks swollen sometimes on one side when it is becoming erect, but never when it is erect.
Sometimes during becoming erect it will bend to one side or the other, but hasn't done that for a while. But it is never in a bent state whilst actually erect.
Sometimes get an urge to pass water, but then there is nothing or very little actually passing.
Fertility - how likely is it to affect me (if it was an STD)?
Have had 3 tests and all have been negative.

Have had a cystoscopy but the urologist said no stricture, but commented on slow urine flow.
Changed urologist and had a bladder pressure test, but again pressure ok, but one urologist said slow flow rate, but the more senior said not a slow flow. It was measured at 7 or so m/sec (I think thats right, haven't got the paperwork to hand).

Is it worth having a retrograde test done for stricture? How accurate are the cystoscopy and which is the preferred test out of all those available?

Is it better to live with the problem, as sometimes it gets worse and other times its better. But sometimes I wake in the morning with abdominal and back pains which is extremely irritating.

Another very irritating thing is sometimes during sex there is a very strange sensation, and result in very quick ejaculation. It is due to the tingling sensation which is there often anyway, but during sex (or masturbation) it is heightened and feels almost as if I really badly need to pass water, except of course I dont.

My first urologist said that he knows there is something wrong, but he doesn't know what.

Is it possible that any "valves" or anything like that is damaged, or is it most likely a stricture and I should just try another test?

A big Thank You!

A burning sensation in the urinary tract, without discharge, is not atypical for chlamydia or any other STD, and variations in penile shape or in the form of an erection absolutely has nothing to do with STD or any other infection.  Your current urinary symptoms--sensation of need to void with little urine--is classical for hyperactive bladder and/or a prostate gland problem, but not chlamydia or any other STD. In any case, if chlamydia had been there at the start but tests for it now are negative, it cannot be causing your continuing symptoms.

With apology, I really cannot help further.  While I am confident you don't have an STD, I cannot otherwise speculate about causes.  In the UK, GUM specialists are trained in all aspects of genital and urinary disorders, and know quite a bit of urology.  Not true for your former colonies to the west, where STD and infectious diseases training and practice are completely seprarate from urology and sexual functioning.

Bottom line: follow up with your urologist, and/or consider visiting your local GUM clinic for a second opinion if you aren't confident the urologist is on top of things.

Good luck-- HHH, MD

I know that sometimes the rate at which you urinate can be psychological. My mate can't **** properly in pub urinals, he has to go into a cubical. Maybe it is psychosomatic? I shut my eyes, think of a waterfall I visited as a kid and just let the **** flow. Probably not much help, but there you go. P.S. Sounds like we get a good deal here in the UK - in America it sounds like you have to pay for everything medical - harsh.

Hi Doctor,

Thanks for the reply.

Is it possible for any STD to leave any permanent damage, other than stricture, for example?

Also how much of a risk of infertility is there if an STD is not treated and resolves itself?

Thanks

Englishman,

I know what you mean, but in my case the actual flow meter considered it slow.

With regards to medical, I pay for health insurance here too, but it still doesn't cover everything. Not sure how the US system works, but at least here in the UK everyone gets looked at, err, at some point anyway.

Cheers.
