Broken Condom During Anal Sex

Hey Doc,

I am a 26 year old male and I am a pretty healthy person overall.  I am uncercumcised if that helps at all in this.

I had sex with an excort this past sunday and the sunday before that.  Both times with the same escort.  The first time she performed unprotected oral sex and then there was protected vaginal and anal sex.  The folowing week I experienced nothing out of the ordinary in terms of sexual disorder.

Then this last sunday I went back to her and again there was unprotected oral sex followed by protected vaginal and anal sex.  This time however the condom broke during anal sex.  I realized almost immediately so I was only inside her for maybe 5 seconds before pulling out.  There was no place to wash up so I went home and within a half hour cleaned up with soap and water.

Then when I woke up on Monday I had some slight discomfort on my penis and that turned to some redness on the top of the head of my penis and also around the opening in front.  This was accompanied by a feeling of needing to urinate frequently as well as a slightly sifferent sensation when peeing but I don't want to call it burning.  Also I noticed that every morning when I wake up I don't feel any discomfort really, I only notice it as I start getting in to my routine for the day.

Yesterday and today the urination seems a bit better and there is less discomfort on the head of my penis and the redness does seem to be going away.  

I don't know the status of this escort but I asked her if she had anything and she said no, not that she had any reason to say otherwise.  

I went to the urologist yesterday and he did an exam and took a urine sample for GC and clamydia but I won't get those results till monday.  

What are your thoughts on all of this?  One other thing to note is that she had a cold the last time and there was a fair amount of mucus during the oral sex.  I have read somewhere that an adenovirus could cause urethritis.  I also got sick with a cold on Monday night, the day after the second encounter.  That is starting to clear up at what appears to be the same time as the redness on my penis.  

I will get my results for these tests on Monday, but should I also go back in a few weeks for HIV and HSV testing?

If the condom broke, you had unprotected sex.  By definition, of course that elevates the risk of infection.  But even so, the actual risk for any particular exposure is low.  Your symptoms are pretty vague; I would be more concerned if you had discharge.  You are right that adenovirus acquired from oral sex can cause urethritis, but usually with more severe symptoms than you describe; and respiratory adenovirus is almost always symptomatic, it's a low risk if your partner didn't have a cold or sore throat.  

I anticipate your gonorrhea and chlamydial tests will be negative.  The risks for HIV and for HSV without symptoms are too low for me to recommend testing on the basis of this event.  But if having negative tests will help relieve your concerns, by all means do it.

I had anal sex and the condom broken it was probably a min or two min before I realize no symptom yet should I be worried about it...

You can ask your doctor if the redness is really causing you anxiety.  But *redness* is not a symptom of any std.  Good luck

One quick question.  The redness on my penis persists still today even though I thought it was getting better.  The frequent urination urges are somewhat better but not completely gone.  I get my gonorrhea and chlamydia results back tomorrow which I assume will be negative because there is not discharge, but what should I ask the doc to do from there.  Would an antibiotic course be appropriate to try and clear up the redness?

Thanks for the response.  Aren't all of those things communicable through oral sex as well though?  With the exception of HIV I suppose.  I am not so concerned about bacterial things because those can be cleared up with some antibiotics.  What I really don't want is HSV.  Something I can never get rid of.

With the abscence of discharge, it's highly unlikely you acquired a bacterial std, which is what should concern you the most.  I'm pretty positvie your resuls will be negatvie.  I'm also sure that you will come up negative for hiv and probably herpes as well.  5 seconds is too short of a time to really contract anything other than bacteria.  Good luck.

HSV has to be massaged in to the skin.  Condoms highly prevent transmission.  So I still think 5 seconds is too short of a time span.  Even if she did have it, she would have to be shedding the virus at the time of the encounter.  With the absence of lesions (that would appear 2 - 20 days after the encounter,) I see  no reason for you to test for it.  Usually the doctor suggests testing if people are experiencing something that *could* be caused by herpes.  But if it will help you sleep better at night, get a Herpes Select type blood test 3 months after the encounter.  Stay safe.

HSV prevention is never 100%, with the exception of practicing abstinence and the assurance of HSV testing of both partners.  Im not one who can tell you what to do or make moral judgements for you, but stay clear of escorts.  Is the 1 hour and $100+ worth the 1-2-3 weeks of worrying and aggrevation?  Im sure you're fine, especially given it was only 4-5 seconds of exposure.  Im sure HHH will tell you that testing is optional for self-assurance.

Thanks for those comments.  As far as escorts go, no, it's really not worth it.  It may take events like this for some of us to realize it, but it most deffinately is not.  If I get out of this without having contracted something I will consider myself lucky and move on.
